Nomination

I understand that some scholarships/fellowship require a nomination from the university. I am aware that if a nomination is required all materials are to be submitted to the Fellowships office in advance of the campus deadline.

In order to assist you with your application process, the Fellowships Office may need to access your personal, conduct and academic information. By signing below, you are giving the Fellowships Office permission to do so, and to share your name and information about your application with appropriate faculty/staff as necessary.
