The Adult Occupational Therapy Teaching Clinic at Puget Sound offers services to members of the community who have physical or cognitive deficits affecting areas of life such as self-care, home management, community participation, and leisure activities. In turn, occupational therapy students in the second year of the program gain skills in evaluating and treating adult participants while under the supervision of a licensed occupational therapist. Sessions are scheduled Tuesdays and Thursdays from February through April, with sessions 45 minutes in length. Participants are responsible for arranging their own transportation.

Common diagnoses treated at the clinic:

  • Cerebrovascular Accident (stroke)
  • Multiple Sclerosis
  • Traumatic Brain Injury
  • Spinal Cord Injury
  • Parkinson's Disease

Physician Referral Form

All participants attending our Adult Occupational Therapy Teaching Clinic are required by state law to have a referral from a physician, physician's assistant, or nurse practitioner.

 

If you feel you or a family member would benefit from our program and the participant is able to commit to consistent attendance please fill out the 2024 Adult OT Referral Form.

Referral forms are now being accepted and reviewed on a first come, first served basis. Prior to scheduling sessions, participants must return all 3 forms, including a health care referral prescribing occupational therapy evaluation and intervention (see p. 3). Once all forms are received, a staff member will call to discuss scheduling options.

All participant referral forms are reviewed by a clinic coordinator to ensure that the teaching clinic experience is applicable for their goals.
